What is a Care Plan?

A care plan is an agreement that we make about the support you want to receive, and what you are expecting from that support. It helps those who are caring for you understand a little bit about you, your medical history, the things you find difficult, the day to day needs, what you want to achieve from your support and the tasks you want us to perform.

Every care plan is unique, because as people we are all distinctive, our personalities, our values, beliefs, backgrounds and cultures are all different; what is important for one person is not the same for another, we all like to do things in different ways. Our care plan’s have been developed over 20 years, to respect your individuality, so that the support you need is tailor made, and just for you. We believe It’s those little extra touches that make all the difference to your support. As time goes by and we get to know you better your care plan will evolve, capturing those things that are important to you. We will also help you think about what you want to achieve from your support, and work together to discover how best this can be realised.

The care plan will include your medical history, details of your GP and other people supporting you and hospital visits. It will include a Care Assessment and Risk Assessments, so that you receive the appropriate support, safely.

The Care Plan will be kept confidentially and safely in our Digital Care Plan App, for the Care Team, carer, home help or companion to have access when needed

 We will formally review your Care Plan usually at, 72 hours, 6 weeks, 6 months and then annually there after. However, we can also respond to any changes or requests between calls, using the Digital Care Plan App, or a telephone call to our
 ‘live team’.
